All groups that are a member of Eastern New York Region must obtain an insurance certificate, and must obtain insurance for events that take place outside of the regular meeting place.

To acquire an insurance certificate, please follow these instructions:

1. Fill out the Group Registration form, or fill out the questionnaire for special events.

2. Fill out the form completely and accurately by typing in all the information.

3. NOTE: If this is a one-time event, fill it out as "meets once".

4. Make sure there is a valid email address filled in on the form, as well as a phone number of a home group member.

5. In the meeting time field, add one hour before the start of meeting and one hour at end of meeting for cleanup.

6. Save the document to your computer. Note: there is no need to print this document.

7. Email completed form to Insurance Liaison.

8. Once it is processed, the Insurance Liaison will then email the insurance certificate to the contact email address entered on the form.

9. You can expect to receive the insurance certificate in 7-10 days.
